BTW, we just posted a CMS care transitions hospital physician&#039;s experience on how simple and clear doctor-to-patient communication can help save patients from readmissions.  It&#039;s a new blog post and pretty eye opening re: how much harm simple miscommunication can cause.  Keep up the great work Konrad!</description>

		<description>This discharge risk assessment tool was designed during a CMS project to reduce avoidable readmission in Louisiana  It is free and available to anyone who wants to download it...CMS is just now pulling together the entire toolkit, but wouldn&#039;t want anyone to have to wait.  CMS found it to be a useful tool, and the readmissions reduction project was successful.
